Licensed Practical Nurse (LPN)

Responsiblities

  • Perform Medication Pouring Training.
  • CPR Instructor Certified or ability to become
  • CNA, DSP, HHA training courses
  • Render general nursing care to clients assigned in their home.
  • Administer prescribed medications, enteral feedings and treatments in accordance with the Physician’s orders and in accordance with approved nursing techniques and facility procedures.
  • Assess and observe clients, record changes of condition and notify the RN Supervisor.
  • Monitor resident vital signs per company policy.
  • Follow documentation policy and procedures.
  • Administer medical enemas, douches and suppositories as ordered by the Physician.
  • Perform catheterizations and obtain urine specimens as ordered by the Physician.
  • Perform nursing summary and quarterly assessments and other documentation as directed by the RN.
  • Ensure that all client needs are being met in accordance with instructions of the Physician.
  • Maintain and clean devices and equipment.
  • Perform routine checks of crash carts and oxygen tanks.
  • Promote positive working relations with staff, families, client representatives, consultants, physicians and other outside agencies.
  • Consults with RN Supervisor on specific nursing issues and interpretation of policies to ensure client needs are being met.
  • Coordinate with the Program Team to ensure client needs are being met.
  • Communicate and promote the company Mission Statement and Values.
  • Advocate for increasing client competencies and independence through best person-centered practices and Social Role Valorization theory application.
  • Promote LifeCourse and life visioning to assure positive life trajectories as identified by the family and supporting team.

Qualifications

  • Must be a graduate of an accredited school of nursing and hold valid current license issued by the State of Indiana.
  • Training Experience
  • In Home Care Experience
  • Must have knowledge of nursing principles, methods and procedures.
  • Knowledge of techniques, theory and practices of professional nursing.
  • Knowledge of, and ability to apply educational and training techniques, ability to plan, organize and direct a variety of nursing activities, ability to plan and supervise the work of others.
  • Ability to establish effective relations with clients, families, Case Managers and co-workers.
  • Ability to support individuals in service throughout the state.
  • Applies knowledge of Person-Centered Planning, Social Role Valorization, and LifeCourse principles to assure wholistic care through individualized and innovative solutions that safeguard risks and promote valued roles.
