Registered Nurse (RN)

Job Description

The Registered Nurse (RN) performs oversight to Manager’s and ISTs to ensure training on medication administration, risk assessments and high-risk plans are addressed by clients and their families. The RN is responsible for any healthcare coordination services provided by LEL.


  • Train Direct Care Staff and Manager’s in medication administration.
  • Make appropriate adjustments and modifications to any high-risk Client plans, as needed.
  • Provide the appropriate follow-up and corrective action planning, as needed.
  • Perform trend analysis on an on-going basis (and, as appropriate, in consultation with other members of management and/or LEL’s legal counsel) to identify safety and/or quality concerns.
  • Report on the status of all on-going medication administration, high risk or health concerns of Clients.
  • Support healthcare coordination in Structured Family Caregiving homes throughout the state including a monthly RN visit to each caregiver’s home to provide an individual physical assessment, review medication administration records, and evaluate health care issues.
  • Advocate for increasing client competencies and independence through best person-centered practices and Social Role Valorization theory application.
  • Promote LifeCourse and life visioning to assure positive life trajectories as identified by the family and supporting team.
  • Conduct training classes.


  • Degree from an accredited nursing school.
  • Licensed as an RN from the State of Indiana.
  • A minimum of one years’ supervisory experience in Home Health Care.
  • Strong interpersonal and relationship building skills.
  • Customer service orientation.
  • Timely implementation of training to assure new hires and veteran staff have the medication administration competencies to delivery high quality care.
  • A positive and pleasant attitude.
  • Ability to work independently and meet deadlines.
  • Strong organizational skills and close attention to detail.
  • Strong problem-solving skills.
  • Ability to support individuals in service throughout the state.
  • Applies knowledge of Person-Centered Planning, Social Role Valorization, and LifeCourse principles to assure holistic care through individualized and innovative solutions that safeguard risks and promote valued roles.
  • Excellent written and oral communication skills.
  • Ability to train Direct Care Staff, families, Clients and Managers.
