Our vision for our individuals is to provide them with ample opportunities to engage within their community.
- LEL provides a variety of activities that help all our individuals to live their great life.
- We promote opportunities to take part in self-advocacy, learn new skills, participate in various hobbies, enjoy Lunch with Friends, participate in Action Clubs, learn a new language, join a walking group, attend Camp events, etc.
- By engaging our individuals in unique and fun ways, our individuals develop natural lasting relationships and friendships and thrive in life experience.
We eliminate boundaries of community integration to allow our individuals to experience limitless engagement opportunities.

WHO WE SERVE
Medicaid Waiver participants in search of integrated and community supports.
The Waivers we provide supports for include:
- CIH (Community Integration and Habilitation)
- FSW (Family Supports Wavier)
- A&D (Aged & Disabled)
- TBI (Traumatic Brain Injury)
- Individuals with additional funding sources seeking a creative approach to support
SERVICES WE OFFER
- RHS – Residential habilitation and support provides assistance to increase independence in daily living skills
- DHI – Day habilitation provides supports to increase presence and inclusion in the community
- Respite – Provides a break for the primary caregiver
- SFC (for both CIH and AD waivers) – Structured Family Caregiving provides an opportunity to live in a family setting and receive the love and support needed to gain independent skills
- PAC – Participant Assistance and Care – provides assistance to increase independence in daily living skills
- Recreational Therapy – is designed to restore, remediate, or rehabilitate through sports, dramatics, arts and crafts, social activities, and other recreation services
- Music Therapy – is designed to focus on the acquisition of nonmusical skills and behaviors by improving and increasing communication skills, social integration, gross and fine motor skills
- Behavior Management – is designed to train, supervise, and assist in the positive expression of emotions and desires to acquire socially appropriate behaviors
- Attendant Care — Attendant care services involves hands-on assistance to allow aging adults or persons with disabilities to remain in their own homes and to carry out functions of daily living, self-care, and mobility
RBHA — Residential-based habilitation service provides training to regain skills that were lost secondary to the traumatic brain injury (TBI) - Homemaker Services — Homemaker services help the individual remain in a clean, safe, and healthy home environment
- FCAR – Family Caregiving Services provide training and education to instruct family members, or primary caregiver about the treatment plans and use of equipment to improve the ability of the family member, or primary caregiver to provide care to the individual.
Ohio Waiver
Services Offered
- Community Respite – Available with the IO or Level 1 Waiver, this service pays for a person to receive support from other caregivers using camps, organized community programs or activities when their regular caregiver is not available or needs a break.
- Community Transition – Available with the IO Waiver, this service can support people as they transition from living in a facility to living in a house or an apartment, with others or on their own. It can pay for one-time household expenses like the security deposit for an apartment or start-up fees for utilities.
- DD Waiver Nursing – People with complex medical needs may require nursing services that are typically provided through Medicaid State Plan services.
- Homemaker Personal Care – Available with the IO or Level 1 Waiver, this service supports a person to be more independent while meeting their daily living needs. Direct service providers help people with household chores and personal care.
- Transportation – Available with the IO, Level One, and SELF Waivers, offers people with disabilities a way to access their waiver services, community activities, and resources when other people or agencies cannot provide transportation free of charge.
- Non-Medical Transportation – Available with the IO, Level One, and Self Waivers, assists a person with transportation to their day services, employment services, or places of employment.
- Shared Living – Available with the IO Waiver, provides an option for someone with a developmental disability to get the support they need while living with a paid caregiver.
- Social Work – Available with the IO Waiver, a counselor or social worker may assist a person with disabilities to become more independent by working on problem-solving, self-help, or adaptive skills, or arrange for counseling or other support services to help ease stress caused by things like work, family, or living situation.
